Member Success Stories: High Touch Member Support
“Mary” is an SSI member with aphasia (unable to speak) as a result of 2 strokes. She is also unable to read or write. She lives alone and has no family in the area.
A MemberConnections Representative was assigned to this member and during her home visits has been able to help her select a Primary Care Provider, schedule an appointment, arrange transportation, and assist with new patient paperwork. She also assists the member by reading and explaining information from her doctor and the health plan to her.
With assistance from the case worker at the PCP office, she was also able to arrange for physical and speech therapy and necessary medical equipment.
