The CentAccount rewards program lets you earn money onto your own CentAccount card simply by doing things that help you stay healthy!  Your card is accepted at most local pharmacies and stores.

Healthy Behaviors

Earning rewards is easy! All you have to do is complete easy healthy behaviors. Magnolia puts funds into your account for completing these behaviors. These behaviors include:

  • Complete a health risk screening
  • Visit PCP in first 90 days of becoming a member
  • Annual Well Woman Tests
  • Annual Breast Cancer Screening
  • Annual Adult Well Care Visit
  • Well Child Visits: Infants (up to 15 months)
  • Annual Well Child Visit: 2-21 years old
  • Annual Body Mass Index Assessment
  • Start Smart – Completion of Prenatal Care Visits
  • Start Smart – Completion of Postpartum Care Visit
  • Controller Medicine for Asthma
  • Completion of all Screenings for Diabetics
  • Annual Diabetic Eye Exam
  • Annual High Blood Pressure Check
  • Completion of Doctor Visit for Chronic Disease
  • Graduation from Disease Management Program

Rewards

Below is a chart that explains the amount that will be put into your account for each behavior you complete.

PCP Wellness Visit in first 90 days of becoming a member $25 1 reward
Cervical Cancer Screening (Pap Smear) $20 1 reward per calendar year (Must be female between 24-64 years old)
Annual Breast Cancer Screening (Mammogram) $20 1 reward per calendar year (Must be female between 42-69 years old)
Annual Adult Well Care Visit (21+ years old) $30 1 per calendar year
Well Child Visits: Infant (0-12 months) $10 Cap at 5 visits
Annual Well Child Visit: (2 – 21 years old) $20 1 reward per calendar year
Annual Body Mass Index (BMI) Assessment $10 1 reward per calendar year
Start Smart – Completion of Prenatal Care Visits (Reward for 3rd, 6th, 9th Prenatal Visit) $15 (up to $45 total) Up to 3 rewards per pregnancy, NOP is required to start visit counter
Start Smart – Completion of Postpartum Care Visit $20 1 reward per pregnancy (Must be 21-56 days after delivery)
Completion of all Screenings for Diabetics (A1c, LDL, Nephropathy Screening, Annual Diabetic Eye Exam) $50 1 reward per calendar year (Must complete all 4 screenings to earn this reward; Must be age 18-75)
Graduation from Disease Management Program $30 1 reward per lifetime (Your Health Coach will decide when you graduate from the program)
