Dear Providers,
We are excited to announce that our annual Provider Satisfaction Survey will be distributed in late July. Your feedback is critical to helping us deliver a best-in-class provider experience and we rely on your survey responses to help us identify opportunities for improvement.
As a token of our appreciation for your time and feedback, all survey respondents who submit their responses by August 9th, 2017 will be entered into a drawing to win one of two Apple iPads.
We are partnering with a third-party vendor, SPH Analytics, to perform this year’s survey. SPH will send surveys to 1,500 randomly selected Magnolia Health providers. The survey is brief and should only take a few minutes to complete.
You can submit your survey responses one of two ways:
1. Fill out the survey and mail it to SPH Analytics
2. Fill out the survey on https://sphawebsurv.com/Provider.aspx and enter the following user name and password:
