Medicare Advantage Providers - Medicare Outpatient Observation Notice (MOON)

Thank you for your continued partnership with Magnolia Health Medicare Advantage. We are notifying providers of the Centers for Medicare & Medicaid Services (CMS) standardized Medicare Outpatient Observation Notice (MOON) (form CMS‐10611).
The MOON is mandated by the Federal Notice of Observation Treatment and Implication for Care Eligibility (NOTICE) Act. The MOON notifies Medicare beneficiaries that they are classified as an outpatient receiving observation services and are not considered an inpatient of a hospital or critical access hospital (CAH).
In accordance with the NOTICE Act, all hospitals and CAHs are required to provide the MOON effective immediately. The MOON and accompanying instructions are available in English and Spanish on the CMS website at www.cms.gov/Medicare/Medicare‐General‐Information/BNI/index.html.
The MOON must include the reasons the individual is considered an outpatient receiving observation services and the implications of receiving outpatient services, such as required Medicare cost‐sharing and post‐ hospitalization eligibility for Medicare coverage of skilled nursing facility (SNF) services. Hospitals and CAHs must deliver the notice no later than 36 hours after observation services are initiated or sooner if the individual is transferred, discharged or admitted.
