Provider Access and Availability Survey

Magnolia Health works to make sure that needed services are available to our members. One of our many goals is to ensure that there is adequate access to healthcare for our members.
In an effort to assess the current availability of services for our members and to meet our State and NCQA (National Committee for Quality Assurance) accreditation requirements, Magnolia Health is requesting that providers complete an Access and Availability Assessment.

The purpose of the assessment is for Magnolia Health to gauge the current state of provider appointment availability and access. The assessment should take no more than 10 minutes to complete and all responses will remain anonymous.
